Great Lakes Health Connect

 

Great Lakes Health Connect (GLHC) is Michigan’s largest health information exchange, with a network of 129 hospitals and 4,000 primary, specialty, and allied care organizations, and a registry of more than eight million unique longitudinal patient records. Through GLHC, Holon was introduced to the Greater Flint Health Coalition in Flint, MI, and was officially selected to help monitor the ongoing water crisis in Flint.

Holon’s technology is being used to alert providers to lead-exposed individuals throughout the continuum of care, and to drive care coordination for these patients. This is especially critical for lead-exposed children. Holon is alerting providers when a lead-exposed child presents in the traditional ambulatory setting, and in the school nursing environment, where providers can use Holon to refer these children to specialists.
